This is really interesting because it is about taking the minetest engine to the limit, I think minecraft servers running on modern hardware (i9 10th gen) can handle around 200 players. Are your friends trying to connect from the internet ? or from a local network ? Also, what IP address are they using to connect ? If you are worried of your clients not downloading the new mod, then you shouldn't, because every time a client joins a server it receives a list of the files it has to download, so if you added a new mod when old players join they will download the new mod automatically.

Yep, that is a bug that has persisted for a long time in Minetest, I guess we will have to wait for 5.4.0. And the other issues you talk about are likely just because your clients have a slow connection to the server, are you running it on a local network or over the internet ?

You are right, you can try on your local server to disable the port forwarding on your router and the server won't appear in the public list. I guess you would have to talk with your VPS provider.

If you are running it on your own computer you have to enter your computer's IP address on the address field and on port leave it on 30000 if you didn't change it.

UPDATE: I will be upgrading the ISP to a better one, so the server will be down from november 27 22:00 UTC for ~24 hours. After that new players will be able to join faster and you will have a little bit less lag.
