I recently came back under a secret account, and noticed that this player named "Warthog" was giving people grief and cheating. They're also known as DaWarthog and DaPurpleHaze.

I can't keep playing here, the server stresses me out and I get snappy at everyone around me due to said stress. So, I'll stick to survival/creative servers from now on. Have fun, everyone.

Maybe just go with totally outlandish names, so you don't have to stick to any established naming schemes.

I was playing 0.57.0, and eternal fire was only created on top of netherrack, not the sides. Is this intentional?

There is a way to spawn entities in generated structures, but it's a bit cheatsy. I found it in the nssb mod, and it works like this: all that happens is that you have a special node where you want the entity spawned, and an amb on it, so when a player gets close, the node transforms into the entity...
