Two things:
1) Use PilzAdam's carts mod, it is more updated and more widely use, and probably more stable.
2) .tar.gz is really similar to zip files, just extract the mod within to the minetest/mods folder.

After extracting rename the folder called carts-master into the name between the brackets [ ] in this case carts .

You seem to be using Windows.

Try an archiver like 7-zip to extract the mod as usual. They are just compressed files like ZIP archives.

If you don't want to go through the hassle, MODSTER can automatically extract and install the mod for you in the correct location.
